Well, I never go hiking. But most likely the stuff that's in my pockets and that already includes a SAK and a lighter even though I don't smoke.

For summer hiking in the midwest I carry:
water
sunscreen/bugspray
small first aid kit with lots of bandaids (kids always manage to need them)
camera

For winter hiking
water
fist aid kit
matches in a waterproof case
leatherman/SAK
solar blankets
extra dry socks
camera

I tend to have a daypack on me for winter hiking/snowshoeing, but only carry a fannypack on summer trips.

Any hiking I do is usually on the North Shore of Lake Superior, where I'm at least an hour away from wherever I'm staying. So, in the backpack goes:

SAK
Water
Lunch (usually crackers and summer sausage, plus some chocolate candy for quick energy)
First aid kit
Camera
Sunscreen and bug spray
Rain poncho
Sunglasses
Kleenex
Hand wipes
Small flashlight (love my Mini-Maglite)
Something to read
Matches in a ziplock bag
Extra pair of clean socks
