As part of the promotion for their first album, The Phoenix Foundation, a tribute band who write and sing songs based on episodes from the 80’s and 90’s hit TV show MacGyver, asked fans of the show to submit ideas for the albums name with the winner receiving an autographed copy of the album along with a life-time pass to all of the bands concerts and their name credited in the CD booklet.
“The competition received 107 entries.” Lead singer Kjetil Friestad confirmed. “and let me tell you ... people out there really had the 4 of us smiling. All those great ideas !!”.
3 of the 107 entrants submitted the name “Troubleshooter” which the band eventually decided as the winning name. “We all loved it - and we hope everyone else will too.” Friestand said. The winning entries came from Louise Owen, Diane Carroll, and Mario Vandermeulen. All 3 will receive prizes once the CD’s have been completed.
The “Troubleshooter” album is set to be officially released on October 10, 2010.

It's a brown/beige cover (they probably liked to stay close to the colors of the first season dvd cover) with in the right corner Phoenix Foundation written and autographs of the band members.
In the lower half of the front cover there's a white blank space with "Troubleshooter" written all over it but just in alcohol marker.
The booklet itself is no big deal. There's no text in it or a word of explanation just pictures of the band. At the back of the booklet again is written Phoenix Foundation in the right corner with the band members name under it. And under that in a smaller font when and where it was recorded and by who it was mixed and mastered and to who the rights are reserved to. On the left side there's just the track list.
The cd itself is the actual Phoenix Foundation logo. The circels, the stripes and the bird. Just as we saw it on the show but it has Troubleshooter under it again and the back is the same thing.
And that's it actually, there's no mention of my name anywhere.
Unless I was mistaken, I tought this was the prize:

Though it's never been said that would be the prize for all three of us but just for the one who was the best or first mentioned.
I can come to the conclusion that, that was the main prize for the winner. And since my name was mentioned last of three winners I guess I won the bronze medal. The second probably get the cd with the pass but also with no mention of her name in the booklet and that the first one gets everything with her name mentioned on the booklet but only on her copy.
At least I was third in a contest. Never happened before and I got the cd but, I won't deny the fact that I was disappointed my name wasn't mentioned anywhere.
I haven't listened to it yet. I did online somewhere before I moved out, but from as soon as I can I will listen to it again.

My copy of the CD was autographed by all four band members on the front, and has a handwritten note to me on the back, thanking me by name for naming the album. Based on the emails I exchanged with them, I was probably the first person to send them the name; but they gave the prize to everyone who had the same idea, which is nice.
The way these are printed, all the booklets will be printed the same -- so there's not going to be any chance of one booklet having any printed text that's left off of the others. However, they indicated that it took them much longer than they expected to finish the album and get it produced; by the time they were done, I suspect they no longer remembered the specifics.
I don't think the band does many shows, but we do get free admission if we're able to attend any of them!

Here's the CD itself -- as MGod said, they used the Phoenix Foundation logo (in brown) for the CD, which I thought was a really cool idea, given that the logo's circular.

And this is the back. I've taken the liberty of blurring out my real name, which was used in the note, and inserting my user name.
The 'booklet' is a single sheet of paper, folded once; the inside was black and white photos of the band, with no text. I suspect that they hit the point where they needed to get the project finished and make the album available, even if it meant having a really basic design for an inexpensive CD sleeve. The emails I got from them indicated that post-production was a lot longer (and probably much more grueling) than they had expected. The band is a sideline, not their profession; I believe they're doing what they do for love.

It's actually 'Andersen' rather than 'Anderson'. They're Norwegian -- not only did the package have Norwegian stamps, the CD came wrapped in pages from a Norwegian catalogue, which was really cool. Variations on the name 'Anderson' are even more common in Norway than 'Smith' is in England.
But for all that: clearly, cosmic synergy.
Incidentally, I noticed on their MySpace page that they have a track you can listen to there that isn't on the CD, "Jack Dalton".
